Question for HAESUNG HOSPITAL

I had my baby three weeks ago at Haesung and never received her birth certificate. How can i request one? I need to change and update her name also. How can i obtain a birth certificate in English with her english name on it? Im american. Can her american name also be written on her korean birth certificate?
